1. Acceptance of Terms

By accessing our website (goodrestmedspa.com), booking an appointment, purchasing a membership, enrolling in an online course, or using any service offered by Good Rest Med Spa (“Good Rest,” “we,” “us,” or “our”), you agree to be bound by these Terms and Conditions. If you do not agree with any part of these terms, please do not use our services.

Good Rest Med Spa is a veteran-owned wellness practice located in Payson, Utah, offering sleep medicine, TMJ treatment, medical aesthetics, massage, and contrast therapy services.

 

2. Services Offered

Good Rest Med Spa provides the following categories of services:

Sleep Medicine & Oral Appliances

• Consultations and treatment for sleep apnea and sleep-disordered breathing
• Custom oral appliance therapy and NightLase laser treatment
• All sleep medicine services are provided under the supervision of our licensed clinical director
• Results vary by individual; no specific outcome is guaranteed

Medical Aesthetics & Laser Treatments

• Laser hair removal (Soprano Titanium), Alma TED hair restoration, and injectable treatments
• All laser and aesthetic procedures are performed by or under the supervision of licensed professionals in compliance with Utah state law
• A consultation and health screening are required prior to certain procedures
• Some treatments require multiple sessions; individual results may vary

Massage & Contrast Therapy

• Therapeutic massage, sauna, and cold plunge (contrast therapy) services
• Clients are responsible for disclosing any health conditions, injuries, or contraindications prior to treatment
• Good Rest reserves the right to decline service if a health concern presents a safety risk

Online Courses & Digital Products

• Good Rest offers online educational courses and digital downloads through its Academy platform
• All digital product sales are final unless otherwise stated
• Course access is granted to the purchasing individual and may not be shared or transferred

 

3. Appointments & Cancellation Policy

We value your time and the time of our clinical team. Please review our scheduling policies below:

• Appointments may be booked online, by phone, or in person
• We request at least 24 hours’ notice for cancellations or rescheduling
• Cancellations made with less than 24 hours’ notice may result in a cancellation fee
• No-shows may be charged the full service fee or forfeit any prepaid deposit
• Good Rest reserves the right to reschedule appointments due to provider availability, emergencies, or circumstances beyond our control

 

4. Payments, Deposits & Refunds

Online Payments & Deposits

• Some services require a deposit at the time of booking to secure your appointment
• Deposits are applied toward the total cost of your service
• Deposits are non-refundable if a cancellation is made with less than 24 hours’ notice
• Full payment for certain packages or memberships may be collected online at time of purchase

In-Office Payments

• Payment is due at the time of service unless prior arrangements have been made
• We accept major credit cards, debit cards, HSA/FSA cards (where applicable), and cash
• Financing options may be available through third-party providers; Good Rest is not responsible for third-party financing terms

Refunds

• Service fees are generally non-refundable once a service has been rendered
• Prepaid package balances are non-transferable and non-refundable unless otherwise specified in writing
• Membership fees are governed by the terms of your individual membership agreement
• Digital course and product purchases are final and non-refundable
• Exceptions may be made at the discretion of Good Rest management on a case-by-case basis

 

5. Memberships

Good Rest Med Spa offers wellness membership tiers with recurring billing. By enrolling in a membership:

• You authorize Good Rest to charge your payment method on the agreed billing cycle
• Memberships auto-renew unless cancelled in writing at least 7 days prior to your renewal date
• Membership benefits are non-transferable and for the enrolled member only
• Good Rest reserves the right to modify membership benefits or pricing with 30 days’ written notice
• Unused membership credits do not roll over unless explicitly stated in your membership agreement

 

6. Health Disclosures & Medical Disclaimer

Good Rest Med Spa provides wellness and clinical services; however, our services are not a substitute for emergency medical care. Please review the following:

• You are responsible for providing accurate and complete health history information prior to treatment
• Failure to disclose relevant medical conditions, medications, allergies, or implants may affect your safety and treatment outcomes
• Good Rest does not diagnose or treat medical emergencies; in the event of a medical emergency, call 911
• Information provided on our website or by our staff is for educational purposes only and does not constitute medical advice
• Individual treatment outcomes vary and are not guaranteed

 

7. Informed Consent

Prior to receiving clinical or aesthetic treatments, clients will be required to review and sign service-specific informed consent documents. These documents will outline the nature of the procedure, potential risks and benefits, alternatives, and your right to ask questions. Proceeding with a treatment following signature of informed consent indicates your voluntary agreement to receive the service.

 

8. Privacy Policy

Your privacy matters to us. Our collection, use, and protection of your personal and health information is governed by our Privacy Policy, available at goodrestmedspa.com/privacy-policy. By using our services, you consent to the practices described therein. Good Rest complies with applicable HIPAA regulations for the protection of health information.

 

9. Website Use

By accessing goodrestmedspa.com, you agree to the following:

• You will not use the site for any unlawful purpose or in violation of these Terms
• You will not attempt to gain unauthorized access to any portion of the site or its related systems
• Content on this site, including text, images, graphics, and course materials, is the intellectual property of Good Rest Med Spa and may not be reproduced without written permission
• We reserve the right to modify or discontinue the site or any content at any time without notice

 

10. Limitation of Liability

To the fullest extent permitted by applicable law, Good Rest Med Spa, its owners, directors, employees, and affiliates shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of our services or website. Our total liability to you for any claim arising out of these Terms shall not exceed the amount you paid to Good Rest for the specific service giving rise to the claim.

Nothing in these Terms limits our liability for gross negligence, willful misconduct, or as otherwise required by law.

 

11. Governing Law

These Terms and Conditions shall be governed by and construed in accordance with the laws of the State of Utah, without regard to its conflict of law provisions. Any dispute arising under these Terms shall be subject to the exclusive jurisdiction of the courts located in Utah County, Utah.

 

12. Changes to These Terms

Good Rest Med Spa reserves the right to update or modify these Terms and Conditions at any time. Changes will be posted on this page with an updated effective date. Your continued use of our services following any changes constitutes your acceptance of the revised Terms.
